What is orthognatic treatment?

The irregular position of the jaws and their underdevelopment often lead to an irregular bite. Orthognathic treatment is a method of treatment in which the combination of orthodontic and surgical treatment corrects the irregular position of the jaws or the underdevelopment of one or both jaws.

During orthognathic treatment, orthodontic preparatory treatment before and after jaw surgery is necessary. Orthodontic preparatory treatment is the straightening of the teeth by wearing braces or teeth straightening grafts until the position of the jaws is corrected, and surgical treatment is surgery on one or both jaws, in which the jaws are placed in a new and correct position.

When is it needed?

Why is it needed?

Malocclusion (improper bite) should be treated for the following reasons:

An improper bite can lead to inefficient chewing, making it difficult to bite and/or swallow food. Poorly chewed food may also contribute to digestive problems.

Bone loss can lead to tooth mobility.

When teeth are misaligned, plaque accumulates more quickly, leading to inflammation of the surrounding gum tissues. Because misaligned teeth are harder to clean, they are also more susceptible to cavities.

Chewing forces are distributed unevenly, causing certain teeth to wear down faster due to excessive load. This increases the risk of developing periodontal disease and makes restoration of these teeth more difficult.

Temporomandibular joint dysfunction (TMJ disorder) may occur, manifesting as pain, clicking sounds, or limited mouth opening.

Bite anomalies can also affect breathing. The person may begin to snore and could be diagnosed with sleep apnea. Due to insufficient oxygen supply during the night, the individual may constantly feel tired and unrested. This impacts other systems in the body as well.

Unfavorable facial features resulting from jaw misalignment can negatively affect a patient’s emotional and psychological well-being.

First step

Diagnostic Examinations

Following a consultation with an orthodontist and a referral to an oral and maxillofacial surgeon, orthognathic diagnostics are performed. These include a CT scan, lateral cephalogram, standardized photos following the orthognathic protocol, a digital dental assessment, and diagnostic dental models.

Second step

Consultation

Before the consultation with the oral and maxillofacial surgeon, all examination results are reviewed, and the patient is offered either a remote or in-person consultation. The doctor evaluates the bite, jaw alignment, joint function, and facial features. Health issues are discussed, and the patient receives general information about orthognathic treatment. A follow-up consultation with an orthodontist may also be required.

Treatment Plan Development and Presentation

The oral and maxillofacial surgeon prepares a detailed treatment plan. This includes the course of treatment, duration, number of visits, and estimated cost. The plan also outlines preparatory work that may need to be completed by other specialists involved in the orthognathic treatment.

Orthodontic Preparation

Preparatory orthodontic treatment using braces or clear aligners typically lasts around 1.5 to 2 years.

Orthognathic Jaw Surgery

A 3D surgical plan is created along with all necessary preparatory tests and procedures to ensure precision. The patient is advised on how to prepare for surgery. The duration of the procedure depends on its type and complexity.

Final Orthodontic Treatment

Once the jaws have fully healed, the final phase of orthodontic treatment begins and typically lasts between 6 to 12 months. The frequency of follow-up visits is assessed. Regular check-ups with a dentist and proper oral hygiene are essential. Long-term retention is crucial to maintain treatment results and involves both fixed (retention wires) and removable (night-time retainers or plates) retention devices.
